I acknowledge that participation in athletic training involves inherent risk. I release the trainer, staff, and facility from any liability for injury, illness, or loss incurred during training sessions. I understand that my child (or myself) participates voluntarily and is physically capable of training.

I consent to emergency medical treatment if necessary.

 

 

 

PAYMENT POLICY

• All sessions must be paid in advance
   

• No payment = no training
   

• Missed sessions are forfeited unless canceled 24 hours in advance
   

• No refunds
   

 

 

 

ATTENDANCE POLICY

​

• Sessions start on time
   

• Late arrivals do not extend session length
   

• Only registered athletes allowed in session
   

 

 

 

MEDIA RELEASE

​

I grant permission for training photos/videos to be used for promotional purposes unless otherwise requested in writing.
